Payment

Full payment is due at time of service. Forms of payment accepted include: cash, debit, major credit cards, and HSA/FSA cards.

Modern Folk Acupuncture is not in-network with any health insurance companies, and I do not bill insurance directly for services provided. If your insurance company covers acupuncture services, I am happy to provide you with a receipt or “superbill” upon request to be submitted to your insurance company for possible out-of-network reimbursement.  Fees reimbursed by insurance are dependent on your individual coverage. Please confirm that your insurance company will reimburse you before booking. 

Cancellation & Rescheduling Policy

Your appointment time is reserved specifically for you. If you need to cancel or reschedule your appointment, please do so at least 24 hours before your scheduled appointment time. Fees for appointments cancelled with less than 24 hours notice and no-show appointments will be charged to the card on file. Changes can be made online using the link in your confirmation email or by emailing sarah@modernfolkacu.com.

All appointments cancelled with less than 24 hours notice will be charged a flat fee of $45 to the card on file. No exceptions will be made for COVID or illness related cancellations made with less than 24 hours notice. Exceptions will be made for medical and family emergencies.

In the event of a missed appointment without any notification of cancellation or rescheduling (“no-show”), you will be charged the full cost of the appointment fee to the card on file. Exceptions will be made for medical and family emergencies.

COVID-19

I am fully vaccinated. Wearing a mask is optional. However, I ask that you exercise common sense when booking and cancel your appointment if you are feeling ill. Please cancel your appointment if you test positive for COVID-19. 

Disposable masks are available in the office if you forget yours and choose to wear one. Please do not hesitate to request that I wear one as well if that suits your comfort level.

Between each patient appointment, treatment rooms are thoroughly cleaned and all linens are changed.

Paperwork, including intake and consent forms before each in-office appointment are completely electronically. Please keep up to date card information on file for easy checkout.
